Missing Participant Reporting Guide

Reporting resources to track missing participants and returned, outstanding, and forfeited checks

There are several reporting tools available in PSW to assist plan sponsors with fulfilling their obligations to search for missing participants and monitor returned and outstanding checks and any forfeited amounts. Download the guide for instructions on using these reporting tools.

• Lost Shareholder & Missing Participants Report
  Lists participants for whom Fidelity Investments® may not have a valid mailing address, participants who are deceased, and participants otherwise deemed missing or lost. The report includes information obtained by Fidelity (but not independently verified) about potential new mailing addresses and contact information (including mail forwarding addresses provided by the U.S. Postal Service).
  
  
• Outstanding Check Participant Notification Report
  Lists email and paper mail notifications sent to participants to notify them of returned or outstanding checks. This report includes the contact information utilized for these notifications.
  
  
• Outstanding Check Forfeiture Posting Summary Report
  Lists participant check amounts posted to the plan forfeiture account, as well as amounts reissued from the forfeiture account.
